After your form is submitted it will be evaluated for approval. If approved, you will be contacted with an estimated time of completion based on current project schedules.

Write to List: When a user submits the form, the information will be saved within SharePoint. At any time, you can easily export the submissions to Microsoft Excel and the data will be displayed as a spreadsheet. This type of form should be used if you are going to be recieving a large number of submissions that you would otherwise be doing data entry for.
Email Generated: When a user submits the form, you will receive an email notifying you of a new submission, and the email will contain all of the information that the user has submitted. The email with the information can be set to send to one or more users. When you recieve the email, you can print or save the submission as a PDF. This type of form should be used if you are receiving requests (like the form on the left) or time sensitive information that you need to be notified of when there is a new request.
