Each student participating in the 2013 New Student Orientation Program must complete a Voluntary Release Participation Waiver. Above, please find the link to this very important waiver. Please print out the waiver, sign it, and bring it with you when you come to your Advising Day in June.

A paper copy of this form will be included in the New Student Orientation Packet being mailed home to you in May. It should be completed and mailed directly to the Student Health Center by July 19, 2013, at the following address:
