Testing Accommodation Form for Students with Documented Disabilities

Students: Fill in the top part of this form and click Submit. A copy of this form will be sent to your professor for approval. You will also be emailed a copy of the form. 

Forms should be submitted by 4:30 pm at least 2 school days in advance of your testing day.  Forms for Monday exams should submitted by the preceding Thursday.

Exams must be started and completed between 8:30 and 4:30, Monday - Friday.

Final Exam Information

You can also use this form to enter your Final Exam information between Tuesday, April 22nd and Wednesday, April 30th.  

When entering your course, please add FINAL after it, for example:

DWC Team B FINAL

ACC 201-001 FINAL 

**Please Note**Final Exam start times are 8:30, 11:00, 1:30 and 4:00 (on Monday and Thursday). The OAS proctors exams until 8:00 pm on Monday and Thursday and 5:30 pm on Wednesday, Friday, and Saturday during Finals Week.
