ATTENTION: The intramural sports office will no longer be accepting paper copies of team roster forms at any of our captains meetings. All rosters must be submitted via email PRIOR TO the designated meeting time. This effort is designed to decrease the amount of paper/ink used by our participants, while ensuring all participants are signing the roster at the game site.
All teams MUST still be represented at the captains' meeting to secure a spot for their team, and submitt the teams forfeit deposit.
Roster forms may be downloaded below. Be sure to fill in all of the required information.
Team Roster Form
Please send your completed roster form to:

If you have an questions, please contact Chris Schmidt, assistant director for intramural sports, at 865-2340.
ALL CAPTAINS’ MEETINGS ARE MANDATORY!
All meetings will be held in the Concannon Fitness Center Meeting Room, which is located on the second floor, unless otherwise noted. All team representatives and officials must bring their PC ID to gain access to the CFC meeting room. These meetings serve as an entry deadline for each sport. No rosters will be accepted after these dates. One representative from each team MUST be present at these meetings (not necessarily the captain). Rosters must be completely filled out and submitted prior to the captains’ meetings. Forfeit deposits ($20 for team sports, $10 for individual sports) will be collected at the captains meeting (CASH ONLY). These deposits will be refunded at the end of the season if the team/individual does not forfeit. Refunds may be picked up any time after the completion of the sport season until the end of the fiscal year. Deposits do not carry over from sport to sport or from year to year! Rosters forms must be downloaded from the intramural website. All information must be filled in for a roster to be accepted. Participants will sign their team roster form prior to playing in their first game.
Note: There is no limit to the number of sports a person may participate in. However, playing for more than one team in any particular sport is prohibited.
