- Math Placement Exam (online at hhtp://angel.providence.edu) completed by June 8, 2009
- Language Placement Exams (online at hhtp://angel.providence.edu) completed by June 8,2009
- Course Selection Survey for the Class of 2013(online at hhtp://angel.providence.edu) completed by June 8,2009
- Building Bridges complete registration form provided in mailing and return to the SAIL office by July 17, 2009
- Student Health Record return the student Health Record on or before July 17, 2009. Please schedule a physical accordingly. Your physician must complete the Student Health Record. For questions related to the health form please contact the center at 401.865.2422.
- Health Insurance If you have health insurance coverage you must complete the waiver at www.kosterweb.com by July 17,2009. If you do not have health insurance, a policy is available for purchase through the College. For questions related to health insurance, please call 401.865.1158.
- Special Programs consider applying for Urban Action, Transformations and Connections.
- Release Forms complete the enclosed forms pertaining to FERPA, Photo Release and Intramural Sports and return to the SAIL office by July 17, 2009.
- Parent Registration Reply Card complete parent registration form provided in mailing and return it with payment by August 7, 2009.
