Please note all forms below are also available for printing from our Forms section which is located on the left hand side of this web page.  If you have difficulty accessing the information, please contact our office and we will help you access the form(s) you need.


Brand New Hire

  • W-4 Form  (Please note if you are claimed by your parents, it is recommended you claim “0” for your tax allowance which will take out your tax liability as you earn your wages)
  • I-9 Form  (Please note if you have a U.S. Passport it will be the only document you need to provide for this form.  If not, a driver’s license or another photo ID and a Social Security Card or a Birth Certificate with a raised seal are required and must be brought in person to the Office of Financial Aid – photocopies are not  acceptable.  International students need to show a social security card and a passport or a social security card and a driver’s license or Providence College ID).
  • Direct Deposit Form  Optional (Please note if you submit this form it is recommended a voided check or checking deposit slip be attached to this form)
    Summer Employment Resource Form  - ( Please note this form is only for students who are working during the summer )

Resident Assistant and Orientation Leader (Academic Year Break-Only)

  • Employment Certification Form  - Required for all Resident Assistants and Orientation Leaders who are working during the academic year break (vacation) timeframes excluding summer.

Rehire

  • If you are returning to the same Federal Work Study (FWS) or Regular Student Payroll (RSP) job you had in the prior year your supervisor should have already placed you on a rehire list and there is no additional paperwork neccessary to complete.

Changing Jobs

Summer Hire
